Apprenticeship Manitoba requires a disclosure form to be on file before they will allow us to verify your operator credentials. Please follow these added steps:
- Complete the Disclosure of Client Credentials form.
- Return the completed form to Apprenticeship Manitoba. (The address and fax number are at the bottom of the form.)
- Mail or fax a copy of this form to BC Crane Safety. (Use the address or fax number on the application form.)
We will follow up with Apprenticeship Manitoba when we process your application.
