Please complete this Application Form and return it to us with:

• A digital photo.
• A clear, legible copy of your certificate.
• US operators only:
  • A copy of your rigger certification.
  • A copy of your proof of pass of the BC Practical Crane Levelling Test.
• Manitoba operators only:
  • A Disclosure of Client Credentials form.
